Evidence in one minute
- Diagnosis is clinical in most patients: history, pitting, Stemmer sign, skin changes and limb shape. In adult unilateral oedema, exclude tumour obstruction; use duplex to exclude DVT and venous disease. Executive Committee of the International Society of Lymphology 2023
- Imaging sequence: duplex first, lymphoscintigraphy to confirm (not standardised), ICG lymphography where surgery is considered; MR lymphography in specialist centres; oil-contrast lymphography only for chylous reflux or the thoracic duct. This is expert opinion (revised 2026-10-09). Avoid biopsy of enlarged nodes in long-standing lymphoedema. O'Donnell 2017
- Always measure both limbs and report excess volume. Ignoring the control limb overestimates treatment effect; 8–12 cm leg segments agree with 4 cm — moderate. Mayrovitz 2007
- Baseline before cancer surgery: 71% of BIS triggers and 88% of tape triggers lay within normal ranges and would be missed without a pre-surgery measurement — low. Then measure about 3-monthly in year 1; the ISL subclinical threshold is a 3–5% volume change. Chen 2025
- Bioimpedance should not be used alone: L-Dex missed most clinically diagnosed BCRL (sensitivity 7.5–25%, specificity 98.5%; 395 women); BIS and perometry identify different women, and most early threshold crossings resolve untreated. BIS-triggered sleeves reduced tape-defined chronic BCRL in PREVENT — low (ImpediMed-designed; revised 2026-10-08). Spitz 2019 · Gillespie 2021 · Ridner 2022
- Tissue dielectric constant (TDC) measures local skin-to-fat water: an inter-arm forearm ratio ≥1.20 is abnormal in 403 healthy women — low certainty for diagnosis (not prospectively validated). Mayrovitz 2026
- Outcome tools: Lymph-ICF-UL for arm QoL; no lymphoedema PROM fully meets COSMIN standards; ultrasound dermal thickness is the only reliable breast measure — moderate. Meilani 2022 · Beelen 2021 · Fearn 2023
Clinical diagnosis
- A quarter of referrals labelled "lymphoedema" do not have it; history and examination diagnose about 90%; lymphoedema is confined to skin and subcutis — anything below the deep fascia is another diagnosis. Mimics include myxoedema, lipoedema, dependency oedema and factitious tourniquet oedema. Greene 2015 · Gloviczki 2024
- ISL stage correlates poorly with QoL, volume, ICG stage and BIS; a lymphoscintigraphic transport index >5 suggests lymphoedema (sensitivity 80%, specificity 94%; 386 limbs). ICG contains sodium iodide — avoid in thyroid disease. Gloviczki 2024
Imaging
- Lymphoscintigraphy is the core confirmatory test; lymphoscintigraphic and clinical staging disagree (4 328 patients). Combined blood-pool and lymphoscintigraphy classify vascular malformations (91%); SPECT-CT helps in genital lymphoedema. Pecking 2008 · Kim 2009 · Ohki 2024
- ICG lymphography stages superficial function and plans LVA but is unstandardised in primary lymphoedema; it shows an upper-arm lymphosome watershed. Brezgyte 2025 · Sakai 2025
- MR lymphography maps vessels ≥0.5 mm; non-contrast MR lymphography can stage disease. CT separates lymphoedema, DVT and lipoedema. Microlymphography shows dermal lymphatic aplasia in Milroy disease. Miseré 2020 · Kageyama 2024 · Monnin-Delhom 2002 · Pfister 1990
- Oil-contrast lymphangiography (SA series from 1965, Rhodesia 1972, Durban 1975) often failed in lymphoedema and caused oil embolism (one death in 277); superseded by lymphoscintigraphy. Van Rensburg LCJ 1965 · Gordon 1972 · Grant 1975 · Abramowitz 1971 · Smith 1966 · Field 1991
Limb volume and shape
- Tape and perometry: frustum (disc-model) tape volumes are valid, and tape and perometer are more reliable than water displacement; the perometer is validated; agreement between methods in established BCRL; PeriKit roughly equals the Perometer for legs; tablet 3D laser scanning correlated closely with tape (pilot; correlation is not agreement). Mayrovitz 2003 · Meijer 2004 · Deltombe 2007 · Stanton 1997 · Ridner 2007 · Louys 2024 · Invernizzi 2020
- Compare limbs: use inter-arm comparison rather than a single-arm frustum; use weight-adjusted change after bilateral surgery; normal inter-leg volume differences exist. Adriaenssens 2013 · Miller 2013 · Teo 2017
- Thresholds: volume difference misses mild BCRL found on examination; a BMI-adjusted 5% criterion over-calls (63%). Stanton 2006 · Mahamaneerat 2008
- Trunk and breast: truncal oedema by modified calipers; ultrasound inter-arm subcutaneous thickness is a practical alternative for arm diagnosis. Roberts 1995 · Giray 2019
Bioimpedance (BIS)
- Performance: flagged BCRL up to 10 months before tape (founding study); L-Dex AUC 0.86 for established BCRL but misses 20% at >+7.1; falls after treatment (ImpediMed-supported). Repeat a raised reading before treating. Detects early extracellular fluid change after gynaecological node dissection, though that pilot's Table 3 shows identical means with p = 0.005 — very low. Cornish 2001 · Fu 2013 · Shah 2013 · Takeuchi 2013
- PREVENT: BIS-triggered compression gave 5.6% v 9.4% chronic BCRL (among those triggered 7.9% v 19.2%) — low: outcome by unblinded tape, tape-triggered patients started at 5–10% on that same tape, and ImpediMed designed the trial. BIS triggers fewer sleeves (20% v 28%, moderate). Subclinical triggers rose from 20% without nodal radiotherapy to 43% with supraclavicular plus axillary fields — low; the comparison uses unequal thresholds. Ridner 2022 · Ridner 2019 · Boyages 2023 · Boyages 2021
- Industry-led documents: the BIS "guideline" was written largely by ImpediMed's CMO and advisers (AGREE II 2/7; L-Dex change ≥6.5, quarterly for 3 years) — cite ISL 2023 for the same schedule (revised 2026-10-09). An ImpediMed-funded review claims a 69–81% cut in chronic BCRL by comparing unrelated studies — very low, do not quote; another BIS review overstates certainty. Shah 2023 · Shah 2021 · Forte 2021
Tissue dielectric constant (TDC)
- Measures local skin-to-fat water to about 2.5 mm, with site- and depth-specific values and no dominance effect; one reading suffices; firmness and water are independent measures; arm TDC varies by site. Mayrovitz 2008 · Mayrovitz 2009 · Mayrovitz 2009 · Mayrovitz 2020
- Thresholds: inter-arm forearm ratio ≥1.20 abnormal (0.74% of healthy women exceed it), 1.15–1.20 borderline, independent of age and BMI; earlier proposals were ≥1.2 (2014) and 1.26 (2009), derived from control variability. Compare ratios, not absolute values (probe, depth, sex and body fat change absolute TDC). Forearm and biceps are usable, the hand too variable; thorax/thorax threshold 1.32. In established BCRL the forearm ratio is about 1.6; after surgery ratios track symptoms and nodes removed (proposed 1.30). Mayrovitz 2026 · Mayrovitz 2014 · Mayrovitz 2009 · Mayrovitz 2007 · Mayrovitz 2016 · Mayrovitz 2018 · Mayrovitz 2015 · Mayrovitz 2019 · Mayrovitz 2019 · Mayrovitz 2015
- TDC detects local change that girth misses (one MLD session: TDC −9.8% v girth −1.5%). Mayrovitz 2008
Questionnaires and scores
- 33 different scoring systems are in use. LYMQOL was validated in 209 patients (MCID about 0.3–0.5); ULFI, SPADI and LLIS are responsive; the Dutch ULL27 is validated; for head and neck, the Patterson scale and LIDS-H&N are best. Sheikh-Oleslami 2025 · Keeley 2010 · Ozkutlu 2025 · Fattah 2026 · Viehoff 2008 · Arends 2023
Before compression
- ABPI is often unreliable in oedematous limbs (cuff size, clinical correlation); UK guidance allows ≤20 mmHg before ABPI if there are no red flags. Keast 2025 · Fletcher 2024
- Lymphoedematous arms are not hypoxic on TcPO2, so a low reading suggests another cause. BNP screening finds occult heart disease before compression. Mayrovitz 2005 · Todd 2011
Gaps and warnings
- Overlapping data: the two 2008–2009 single-reading TDC papers report the same 10 patients, and the 2009 and 2014 threshold studies probably share participants — count each once. Mayrovitz 2008
- Much of the BIS and TDC evidence comes from single groups with commercial or developer links (ImpediMed; Mayrovitz).
- No prospective validation of any TDC threshold; the 2012 ISL proceedings are not held.
For the therapist
- Do: measure both limbs and report excess volume; use the same method and protocol every time (tape with the frustum formula is adequate and valid); record the Stemmer sign, pitting and skin changes.
- Choose an outcome tool: Lymph-ICF-UL for the arm (best measurement properties); LYMQOL (smallest important change about 0.3–0.5); Patterson scale and LIDS-H&N for head and neck.
- Stop: relying on BIS alone or on a single raised reading.
- Certainty: limb-measurement agreement moderate; TDC thresholds low (not prospectively validated).
For a vascular surgeon in South Africa
- The SA referral pathway proposes pre-treatment limb measurement for every newly diagnosed breast cancer; a tape measure and the frustum formula are adequate. Marco 2014
- Lymphoscintigraphy is the realistic confirmatory test; ICG and MR lymphography are limited to a few centres.
