Evidence in one minute
- What it is: bilateral, symmetrical, painful fat of the legs (and sometimes arms) that spares the hands and feet (a "cuff"), with easy bruising and a nodular texture. It starts at hormonal change and occurs almost only in women. In pure form the Stemmer sign is negative and early lymphoscintigraphy is normal; pitting or a positive Stemmer sign suggests lipo-lymphoedema. - Prevalence depends on the definition: UK minimum 1 in 72 000 v German ~10–11% of women; the 10% figure is unproven. Kruppa 2020 · Funke 2023
- Cause unknown: hormonal theories (oestrogen-receptor imbalance, AKR1C1, CAV1) remain laboratory hypotheses, and no marker separates lipoedema from obesity. Lüchinger 2026 · Cifarelli 2025
- Conservative care: compression for symptoms and lipo-lymphoedema, movement, weight management for co-existing obesity, and psychological support. Conservative care reduces volume by 5–10%; diet and exercise do not remove lipoedema fat. Kruppa 2020
- Liposuction — first randomised data (LIPLEG, 12 months): pain fell by ≥2 points in 68% v 8% with CDT, with more adverse events (196 v 32). Diagnosis and phenotype
- Clinical features in 138 women: symmetry, spared feet, pain (92%), bruising and disproportion; knee osteoarthritis is common. BMI overstates obesity (64% v 32% obese by waist-to-height ratio). Forner-Cordero 2021 · Melican 2025
- Tests that help: capillary fragility (bruising) testing separates lipoedema from obesity; TDC is higher in leg lymphoedema than in lipoedema; CT shows no oedema in lipoedema. Szolnoky 2017 · Tuğral 2023 · Monnin-Delhom 2002

- Spanish clinic cohort (1 803 patients): mean BMI 28.6, one-third normal weight, nearly half Schingale stage IV–V. Its near-universal comorbidity rates (hypermobility, "intestinal permeability" 99%, "ovarian dysfunction" 76%) come from unvalidated in-house questionnaires and examiner-devised signs, with arithmetic errors and an undeclared institutional interest — very low; do not quote them (revised 2026-10-09). Treatment

- Diet: ketogenic diet −7.9 kg and −1.1 pain points from uncontrolled before–after data only — very low; no proof it beats other weight-loss diets. Diet, compression and aquatic exercise give partial relief (very low). Amato 2024 · Vazirnia 2026
- Liposuction: LIPLEG (405 women, 2:1 liposuction v CDT, publicly funded) has an endpoint-timing asymmetry built into its protocol. Its weaknesses are a poorly documented CDT arm, longer follow-up in the surgical arm and unblinded patient-reported outcomes. The figures held are second-hand: the critique's author works at a conservative lymphology clinic and its translation was funded by a compression-industry federation, and several criticisms are contradicted by the protocol (7-month standardised CDT run-in, taped scars for assessor blinding) (revised 2026-10-09). - Varicose vein surgery rarely helps the swelling in lipoedema or lymphoedema legs. Földi 2000
Gaps and warnings
- Review quality (JBI): Kruppa 2020 6/6 is the most balanced source (liposuction evidence observational). Shavit 2018 2/6 calls liposuction the treatment of choice on case series, and a co-author performs it (undeclared). Fife 2010 3/6 has a good differential table but calls lipoedema genetic while stating that no genetic data exist. A 2026 review from a liposuction practice (undeclared interest; AMSTAR 2 critically low) strongly recommends tumescent liposuction on observational evidence — very low (revised 2026-10-09). - The Lipedema Foundation guide is an accurate checklist, but its claim that bariatric surgery has "limited to no effect" is unsupported; its research reviews are foundation-funded. Lipedema Foundation 2024
- A German liposuction surgeons' guidebook concedes "there is simply almost no" evidence in lipoedema, rejects "we cure lipedema" advertising and paid bloggers, abandons >20 L "megaliposuction", and calls Germany's stage-based insurance cover arbitrary. Jandali 2022

For the therapist
- Do: confirm the diagnosis (symmetrical painful fat sparing the feet, negative Stemmer sign); use compression for symptoms and any added lymphoedema; support movement, weight management and psychological wellbeing.
- Stop: promising that diet or MLD will remove lipoedema fat.
- Before liposuction: decongest any fluid component and record a baseline.
- Certainty: conservative care and liposuction both low to very low.
